DLCAS.com Transcript Request Form

If you are a current DLCAS.com student and are ready to make an application with your local board you will need to request a transcript from us.  To do so, please fill out the form, below, in its entirety, so we may start pulling your records and populating your request.  We will do our best to get your transcript over for your review as quickly as possible, but it could take up to 48 business hours depending on the volume of requests.  We thank you in advance for your patience while we work on your request and if you have any questions in the meantime you can reach us during our normal business hours Monday - Friday 9am-4pm CST at 866-431-4240; or you can send an email to information@dlc.email.
